After Requesting an Appointment
You will need to submit before your appointment with the doctor:
- Please submit any pertinent medical records, we need the documentation of the qualifying condition (see list in the appropriate state web site).
- List of medications (all medications, including over the counter supplements/herbs).
- Copy of your state ID or driving license of the state you are going to apply for the certification.
- For PA residents, we would need you to call ahead of time to the medical marijuana program and get an ID number. We would need that number to be provided to us to find you in the system and allow us to make the certification.
